
RMBC Preparation for Adulthood Team (PFA)
Who are we?
The Preparation for Adulthood Team is part of Rotherham Metropolitan Borough Council and is based in Riverside House. The team is a part of the Adult Social Care Service & works with young people from the age of 14 to 25 years old.
What is Preparation for Adulthood?
Preparation for Adulthood is the period of time when young people moving from childhood into adulthood.
This can involve changes in many areas of life, such as:
Preparation is a journey, not a single event. It’s about planning and preparing for the future, so young people can live as independently and confidently as possible.
Who do we support?
The team supports young people who have additional needs between the ages of 14 - 25 and offers advice, guidance and support both to the individual & the people who care for them.
How can the Preparation for Adulthood Team support you?
Good support in Preparation for Adulthood can be the key to building a strong foundation for a young person’s adult life.
Preparing into adulthood can be exciting and sometimes worrying. You may have questions, like:
· What happens when I leave school or college?
· Will I get a job?
· Can I leave home?

A worker can help you identify and achieve your goals.
A Preparation for Adulthood worker can help you connect to available support.
Which other teams do we work with?
The team work with a range of people who can support with Preparation into adulthood.
This might include:
· Children’s social workers
· Teachers
· SEND workers
· Employers
· Health professionals
· Voluntary and independent sector providers.
How do we do it?
A Preparation for Adulthood worker will arrange a meeting with you (& anyone you would like to be there with you). We will help you and your network through the assessment process.
You and the Preparation for Adulthood Team worker will talk about what you are able to achieve, both with or without support, and what your aspirations are.
What then?
A Preparation for Adulthood worker will work with you and your network to put together a plan to support you.
This plan will show how the needs and outcomes identified in the assessment will be met.
This plan is unique to you.
Also: Financial Assessments
You will receive a financial assessment to work out if you need to make a financial contribution.
The financial assessment will also help you to maximise your income
Is your plan working for you?
Once the plan is in place, you and the Preparation for Adulthood worker will review your needs and outcomes and what is working well & if anything is not working well.
Your plan may continue with the Preparation for Adulthood Team up to the age of 25.
