Summary of service
The Young Adult Transitional Team provides information, support and practical help to young people aged between 16 and 25 who have a physical disability. The aim is to bridge the gap between childrens and adult services and to empower young people where possible to take responsibility for their health and wellbeing. The team is made up of a physiotherapist, occupational therapist and a social services officer. They will see young people at the team base, Clifton, in young peoples homes or any other convenient location.

We aim to assist young adults to achieve goals that are relevant and important to their life.
We work with clients aged between 16-25 with the aim of bridging the gap between paediatric and adult services, empowering clients to take responsibility for their health and well being.
The team consists of a part time Physiotherapist (25 hours), part time Occupational Therapist (22 hours) and full time Social Service Officer (37 hours).
The team can visit clients in their own homes or other convenient locations.
